- The fees listed above include the appliance insertion, active treatment, and retention fees.
- These fees DO NOT include the records fee which is an additional $129
- If treatment becomes extended due to excessive appliance breakage or loss, or chronically missed appointments, additional monthly fees will be charged pro-rated at $123 per month.
- If you have selected a monthly payment option, payments are due regardless of whether or not we treat the patient during that particular month. Payments are not related to the number of times in a month that a patient is seen, they are merely a method of budgeting your total fee over time.
